We are excited that you are choosing to establish care with us! Please call (925) 254-3805 with your insurance information to make your new patient appointment. Then Fill out the new patient forms and bring it to your first visit. If medicare, please also fill out the Medicare Secondary Payer Questionnaire.

Please note that only Dr. Jessica Tsuei is currently taking new primary care patients. And Dr. Yenjean Hwang is taking only new infectious disease consultations
