    We encourage new or existing patients to sign up for our online patient portal, mychart with Brown and Toland.

    Once established with the office, you will get an activation code to the provided email address to set up a mychart profile.

  • Appointments

    Patients are seen daily on an appointment only basis. Walk-ins are not accepted.

    There will be a fee for any cancellations within 24 hours of appointment time.

    Please keep in mind that patient emergencies do occur. We make every attempt to have you be seen at your scheduled appointment time but unexpected delays can occur.

    We appreciate your patience.

  • Prescription Refills

    If your prescription has expired then most likely you are due for an office visit. We will only renew prescriptions if a patient has been seen for that medical condition within the past 12 months.

    If you have an existing prescription, but are out of refills and have been seen in the past 12 months, please call the pharmacy and request a refill even if it shows you have no refills left.

    **Each provider has their own policy regarding controlled prescriptions.

    There is a 24-48hr turn around time for most prescriptions. This can take longer if your insurance requires prior authorization.

  • After Hours/Emergencies

    After the office is closed, a physician is on call for urgent problems that cannot wait until the next office day. Please note, you may be billed through your insurance depending on the situation.

    Patients with serious medical emergencies, including life or death situations, should dial 911 or proceed directly to the nearest emergency room.

    Note: the office does not refill medications after hours or on weekends.

  • Vaccination Hours

    You do not need to see the doctor to receive vaccinations.

    Vaccines can be scheduled with the medical assistant.

    Hours: Tuesdays, Wednesdays, Thursdays between 9-11AM, and 2-4PM. Fridays are limited to the morning time slots.

  • We take most major insurance. Our office does not take Medi-Cal or Medi/Medi.

    To confirm acceptance of your particular plan, please call the office at (925) 254-3805.

    When you receive care from our practice, we’ll be happy to submit the claim to your insurance company on your behalf. You’ll be expected to pay for any applicable co-payments, deductibles, co-insurance, or non-covered services in a timely manner.

    If you don’t have health insurance for your primary care visits, you may pay out-of-pocket, in full, at the time of the appointment.

  • Free easy parking steps from the door.

  • We have the Abbott ID Nucleic acid testing with a turn around time of as early as 15 minutes. The technology used is different and more accurate than the home antigen test.

    We can perform a nasal swab and send to the lab for covid PCR testing if requested. TUrn around time depends on the individual labs. Mostly 48-72hr.

  • You will have to fill out a medical record release form. Typical turnaround 7-10days. Fee $50

  • We kindly ask that you please let us know 24 hours in advance if you need to cancel or reschedule your appointment so that we can provide care to another patient.

    A cancellation fee may be applied if you cancel within 24 hours or no show.

    For physicals, standard cancellation fee of $100 will be charged.
